DR8495 Tax Check Authorization, Waiver, and Request to Release Information Diagram of Licensed Premise. A licensed premises is specified in an application where alcohol has been approved to be served, stored, and consumed. The licensee must submit a diagram of the proposed license premises which includes dimensions, bar location, entrances/exits, and other pertinent information to the type of license applied for. It can be hand-drawn on an 8 1/2" x 11" paper. Possession (e.g. Lease/Deed/Contract). A lease, deed, or contract (in the name of the applicant) is required to show that the licensee has control (possession) over the geographic location or address. The document must be a minimum of 1 term from the date of filing.

How are Fingerprints taken? Due to Senate Bill 17-189, the procedure for fingerprinting has changed. Hard copy cards, in most cases, are no longer accepted. The Colorado Bureau of Investigations (CBI) has implemented a new process that began September 4, 2018, called the Colorado Applicant Background Services (CABS). There are state-approved vendors around Colorado, and appointments are available online. In order to get results, you need to contact CBI to be issued your CONCJ #. In addition to the State required documents, you (as a local municipality) can request additional information from your applicants. This includes zoning verifications, financial questionnaires, additional affidavits, etc. Once a completed application has been filed with your office and the appropriate fees have been paid, you must schedule a hearing with your liquor board or hearing officer. The hearing cannot be sooner than 30 days after receipt of the application. At the hearing, your applicant must establish the needs and desires of the neighborhood. Once approved by your board/hearing officer, the approval is sent to the State (if the applicant did not pay for concurrent review, you send the entire application with local approval to the State). The State will send you a license (or corrections if needed) when their review is complete.
All liquor licenses are renewed annually. The State sends out semi-populated DR8400 to the most recent licensee's mailing address on file. If the licensee doesn’t receive it or loses the document, they can complete a blank form found on the State’s website. The DR8400 must be filed with your office 45 days prior to expiration. If the applicant fails to file on time, there is a $500 late fee charged by both State and local authority. There is a $25 per day fee for every day it is late past 90 days after the expiration date. In addition to the DR8400 every applicant must submit a DR8495 Tax Check Authorization, Waiver, and Request to Release Information. Failure to complete the DR8400 or turn in the DR8495, will cause a delay in issuing the new license.
